We've compiled a list of required documentation veterans will need when applying for educational benefits at SIU Carbondale.
Chapter 30 (Active Duty) MGIB
- DD-214 Member 4 copy
- Documentation of $600 Buy-Up. Verification can include information on the DD214 Member 4 copy, LES statements, and/or a receipt.
- Proof of college fund if applicable.
Chapter 33 (Post 9/11) GI Bill®
- DD-214 member 4 copy(ies) documenting all active duty service after 9/11/01.
Chapter 1606/1607 (National Guard/Reserve) GI Bill®
- N.O.B.E (Notice of Basic Eligibility)
- Kicker contract
- DD-214 member 4 copy W/proof of deployment. A retirement point summary may be requesting if period of active duty cannot be documented (1607 applicants only).
For Transfer Students to Receive GI Bill Benefits at SIU Carbondale Documentation needed for IL State Va benefits
- Complete VA form 22-1995 (Request for Change of Program/Place of Training) The VA form can be found at benefits.va.gov/gibill/ and must be completed to receive benefits at your new Primary School.
Illinois Veterans Grant (IVG)
- DD-214 member 4 copy W/proof of active duty service and Illinois residency upon entry of and separation from military service.
- Must have Honorable Discharge.
- For National Guard/Reserves, if you had been federally activated for service (Global War on Terror, Operation Enduring Freedom, etc), you will need all DD 214’s documenting periods of Active Duty Service.
Illinois National Guard Grant (ING)
- Must have 1 year of Illinois National Guard service and still be actively drilling.
- Information and Applications can be found at ISAC.org. Must apply for each academic year. *Note: Deadline dates for each term. SIU School Code is: 060.
